What is Eye and Eye Contour Aesthetics?
Eye and Eye Contour Aesthetics: It is not very correct to evaluate the eyelids separately from the tissues around the eyes. The eye and eye contour, even the whole face, are aesthetically inseparable parts of a whole. Therefore, it would be more accurate to use the term eye and eye contour aesthetics, not just eyelid aesthetics.
Eyelid aesthetics (blepharoplasty) includes many procedures such as brow lift, mid face lift (SOOF lift), wrinkle (crow’s feet) removal around the eyes, lower eyelid (tear trough) filling. All of these procedures can be performed on a single patient or only one or a few of them can be applied according to the needs of the patient.
What are Eye and Eye Contour Aesthetic Procedures?
Eye and eye contour aesthetic procedures can be divided into two as surgical or non-surgical. In surgical procedures, local anesthesia can be applied, as well as sedation (the patient is half asleep) or general anesthesia. In non-surgical procedures, a superficial local anesthesia will be sufficient.
Surgical incisions can be made with a scalpel or radiofrequency device which has a lower risk of bleeding, in natural folds or in areas that are hidden to avoid scarring later.
Upper Eyelid Aesthetics
Upper eyelid aesthetics is the process of excising the excess, sagging skin on the upper eyelid to give the patient a more youthful and refreshed appearance.
In line with the patient’s needs, the patient can be treated with the excision of the subcutaneous muscle (orbicularis) tissue, herniated and prominent orbital fat tissue, and even the fixation of the sagging tear gland in its old place.
Lower Eyelid Aesthetics
Lower eyelid aesthetics mostly involves making a hidden incision through the conjunctiva (the red membrane tissue behind the lid) and removing the herniated and bulging fat tissue (treatment of lower eyelid bags and swelling) or transposing it to the depressed areas under the lid (treatment of lower eyelid depression and hollowness).
In this surgery, skin removal should be avoided unless it is mandatory, and if it is really necessary, a minimal skin spindle should be removed.
Lower eyelid aesthetics is a surgery that requires more attention than the upper eyelid and should be performed by surgeons with serious experience in this field.
Removing too much tissue from the upper eyelids can make it difficult to close the eye and cause serious eye problems. However, the positive effect of gravity on the closure of the upper eyelids reduces the risk of complications.
However, the opposite is the case in the lower eyelids. Since the lower eyelid also carries the cheek load, an overly aggressive surgery combined with the negative effect of gravity can lead to conditions that can seriously threaten eye health, such as pulling the lower lid down and turning the lid outward.
In lower eyelid aesthetics, if the lower lids and the suspension elements (ligaments) holding them up are very loose, it is necessary to strengthen the suspension ligaments (cantoplasty / canthopexy) in the same session.
In fact, if the cheek tissue has been displaced downward too much, a mid-face lift (SOOF lift) should be added to the procedure. Otherwise, it is inevitable that the lower lids will turn outward and pull down.
Elevation of the lateral angle of the eye (Cantopexy / Cantoplasty)
(Cat-eye)
Cantopexy or canthoplasty surgery, also known as cat eye surgery, can be added to upper or lower blepharoplasty surgery or can be performed as a completely independent procedure. For aesthetic, youthful and dynamic looking eyes, normally, the outer angle of the eye should be 1-2 mm higher than the inner angle.
Due to structural reasons or trauma, aging, etc., this structure may descend downwards and cause a tired and sad expression. In such patients, the lateral angle can be elevated with a separate minimal skin incision or during eyelid aesthetics without the need for an additional incision.
Temporal Lift (Bella eyes)
The Temporal lift (Bella eyes) procedure, in which the outer angle of the eye and the outer eyebrow area are elevated, is performed completely discreetly with minimal incisions concealed in the hairy area. This procedure allows the external angle of the eye and the outer brow area to be raised without any incisions around the eye.
Brow lift
Over time, with the negative effect of gravity, the eyebrows can be displaced lower than they should be. Although the heights at which the eyebrows should stand are different in men and women, a lowered eyebrow can cause more skin to pile up on the upper eyelid and thus cause difficulty in opening the eyelids. A low eyebrow can easily be overlooked by a physician who does not have enough experience. Therefore, the skin piled on the upper eyelid may appear more and as a result, it may cause more skin to be removed than necessary in upper eyelid aesthetics.
Therefore, when planning upper eyelid aesthetics, eyebrow levels should be evaluated very well, if there is a low eyebrow, the eyebrow levels should be adjusted first, and then the excess skin tissue remaining on the upper eyelid should be excised. Eyebrow level adjustment and upper eyelid aesthetics can be performed in the same session and at one time, but it is necessary to have a serious oculoplastic surgery experience for this.
Non-surgical procedures
Fillers
We can categorize the wrinkles on our face in 2 main groups as dynamic (caused by facial expressions) and static (independent of movements, constantly existing). Since dynamic wrinkles are due to the movements and contractions of the mimic muscles, the most effective method for their removal is toxin application.
However, since there are permanent folds in the skin itself in static, that is, permanentized lines, toxin does not benefit in these. The most suitable method for these lines is filler applications. Thanks to the fillers applied to the curled areas of the skin, the lines disappear and your skin gets a brighter appearance.
Thread lifting
Sagging occurs in our skin with the effect of gravity and aging. These sagging can be reversed to a certain extent with the thread lift method.
However, it is necessary to know that this effect is limited and temporary. There are threads made of different materials used in the thread lift method. The general principle of all of them is to pull the sagging skin higher and fix it to the underlying deep tissues thanks to their serrated structure.
Threads of different structures have different times to melt under the skin and lose their effects. As these threads dissolve under the skin, fibrosis develops instead, which allows the suspension effect to continue for some time.
Nowadays, longer lasting and more permanent silicone-coated threads, produced by a French company and therefore called “French sling”, are used more frequently.
With this method, the lifting effect lasts approximately 5-6 years. However, although these applications are called non-surgical, since permanent materials are implanted under the skin, they should be applied by experienced people with the utmost attention to sterilization rules. Otherwise, infections and unwanted permanent negativities may be encountered.
